Power of attorney and instruction to create a business listing on SocialImpactMap

1. Scope

I hereby authorise and instruct SocialImpactMap, on behalf of the business named above:

  • to create an account on socialimpactmap.com and a business listing,
  • to enter the following details for it: name, address, category, description, website, logo, photos and a description of the business's social engagement,
  • to upload proof I provide (e.g. donation receipts, official donation confirmations, proof of partnerships),
  • to submit the listing for review and to answer any questions about it.

2. Texts and content

SocialImpactMap may word the description of the business and of its social engagement at its own discretion. It bases them on my information and on publicly available information, e.g. my website or social media profiles. I have no claim to any particular wording.

SocialImpactMap will only present engagement that is supported by my information, proof or public sources. I am responsible for the accuracy of the information I provide.

3. Access by email, review and changes

Once the listing has been created, I will receive an email at the address given with a link to set my password and access my dashboard. I undertake to check the listing promptly and to change or add to it where needed. I can change or delete the listing myself at any time.

The listing may be published as soon as SocialImpactMap has reviewed it.

4. Charitable cause and donation

My details in this form on organisation and area form part of this authorisation.

SocialImpactMap donates €20 in my name to the non-profit organisation I have named. If I name no organisation, SocialImpactMap donates to a verified non-profit organisation of its own choice – from the area I have chosen, if any. SocialImpactMap bears the €20; this costs me nothing. As the donation is paid by SocialImpactMap, I do not receive a donation receipt for it.

5. Logo and images

I agree that SocialImpactMap may use my logo and images from my website or social media profiles for the listing. I confirm that I hold the necessary rights. In addition, my logo and images may also be used to promote the SocialImpactMap platform; I can withdraw this consent at any time.

6. Costs

Creating and publishing the listing, as well as the donation, are free of charge for me. There is no subscription and no contractual commitment.

7. Terms of use and data protection

SocialImpactMap's terms of use apply (socialimpactmap.com/terms). My data is processed in accordance with the privacy policy (socialimpactmap.com/privacy) solely to create and manage the listing (Art. 6(1)(b) GDPR).

8. Duration and revocation

This authorisation applies from the moment this form is submitted. I can revoke it at any time without giving reasons, in text form (e.g. by email to info@socialimpactmap.com). Upon revocation, or once I have taken over the account myself, SocialImpactMap will make no further changes on my behalf. On request, the listing will be deleted.

9. Authority to represent and signature

I confirm that I am authorised to represent the business named above in making this declaration. I sign this authorisation electronically by entering my name, ticking the confirmation and submitting the form.
